Mental Health Survey

The Carver County Adult Mental Health Initiative (AMHI) and the Carver County Mental Health Advisory Council, or Local Advisory Council (LAC), have combined efforts to create a mental health needs assessment survey. If you are a resident of Carver County, aged 18 and older, and you have or have not accessed mental health services in the county, we ask you to please take a few minutes to complete this confidential survey.

Through this survey, the AMHI and LAC hope to gather pertinent information about mental health needs in our county in order to improve the availability of and access to adult mental health services, and to guide future planning of services. Your participation is completely anonymous and will not in any way affect the county mental health services you may currently receive or may choose to receive in the future.

The survey starts September 15, 2017, and will be available until October 31, 2017. It can be accessed online at If you prefer, a paper copy, along with a self-addressed stamped envelope, will be made available upon request.
